Safety & Consent
Before your child's first ride, we complete a short set of forms together. They exist for one reason: so every ride is safe, authorized, and documented.
Forms Every Family Completes
We walk you through each one — nothing is buried in fine print.
Transportation Consent
A parent or legal guardian authorizes Scuber to transport the child, listing routine destinations such as school, daycare, or activities.
Emergency Contact Information
At least one alternate adult with phone number, plus any medical notes or allergies we should be aware of during a ride.
Child Information Form
Name, age, grade, seating needs (booster or seat belt), school details, and anything that helps your child feel comfortable.
Cancellation / No-Show Policy
Cancel at least 12 hours ahead for a full credit. Same-day cancellations and no-shows may be charged for the reserved ride.
Liability Acknowledgment
A plain-language acknowledgment of service terms, insurance coverage, and the responsibilities of both driver and guardian.
Authorized Pickup / Drop-Off List
Only adults you name in writing may release or receive your child. Photo ID may be requested at any hand-off.
Your Child's Information Stays Private
Family details are collected only for the purpose of transporting your child safely. We never sell, share, or post information about your child, and records are kept securely and limited to the driver.

- No photos or names of children posted publicly
- Records shared only with authorized guardians or emergency responders
- Forms removed on request when service ends
